If you are living in Spain and do not have access to public healthcare through the usual route, the Convenio Especial may be an option for you.
The scheme is based on a national framework, but the application is handled by the relevant regional health authority according to where you are empadronado.
The Convenio Especial is a public healthcare agreement that allows eligible people to access the basic public healthcare system in Spain by paying a monthly fee.
It is intended for people who are not insured or covered as a beneficiary through the normal public system.
In general, applicants must be resident in Spain, must not have access to public healthcare through another route, and must show at least one continuous year of effective residence immediately before the application.
They must also be empadronado in the area where they apply.
The cover is for the basic public healthcare service package.
It includes the normal basic healthcare services of the Spanish National Health System, but supplementary items such as outpatient prescriptions, orthopaedic items, dietary products, and non-urgent medical transport are not included in the same way and may be fully payable by the patient.
The standard monthly fee for the Convenio itself is €60 per month per person if under 65, and €157 per month per person if aged 65 or over.
These are the national base amounts.
The law also allows autonomous communities to increase the fee if they add extra complementary cover.
Because the scheme is administered regionally, the forms, supporting documents, and submission route can vary depending on where you live.
That is why it is important to check the correct regional procedure before applying.
